Embrace Health & Rehab accepts and is a "participating provider" with Medicare and Health First insurance (commercial and HF Medicare)

Please note: Cigna Allegiance through Health First is not Health First insurance and is still considered out of network

We are out-of-network with all other insurance providers.

Superbills are provided after each session for you to submit to your insurance company for potential reimbursement.

HSA/FSA Payment Accepted

Each visit is $175

Physical Therapy visits are typically 55-60 minutes each and visits are scheduled once every 2-4 weeks or more frequently as needed

Visit Packages

4 Visits: $660 ($165/visit)

8 Visits: $1,280 ($160/visit)

12 Visits: $1,860 ($155/visit)

Packages are non-refundable
